►Protecting 你r Privacy

Salem Regional Medical Center is committed to protecting your confidential health information. 我们的 Notice of Privacy Practices explains how we will safeguard your privacy.

当你在这里的时候, 你的家人, friends or other members of the public may request your room number and general medical condition, if you gave permission for the release of this information at the time of your admission. Please note that this information can be obtained by calling our 患者信息 Desk at 330-332-7194, or by inquiring in person in the Main Lobby. To protect your privacy, specific details about your illness or treatment will not be provided.

If you prefer that this information not be released, you must indicate this at the time of your admission.

During your hospital stay, your doctor(s) and nurses will try to answer your questions and talk to you about any concerns you may have throughout your hospital visit.

Before you leave Salem Regional Medical Center, please read the "Be Prepared to Go 首页 Checklist" in your patient folder, to prepare you and 你的家人 members for your care transition after you are discharged from the hospital.

Advance Directives are legal documents that provide instructions about your health care wishes, in case you are unable to speak for yourself. Ohio State Law recognizes 生活将会s and Health Care Powers of Attorney as Advance Directives.

Salem Regional Medical Center respects the right of each individual patient to make decisions concerning his or her medical care, including the right to prepare Advance Directives. If you have questions regarding Advance Directives or would like assistance in preparing your Advance Directives, 打电话给医院 Case Management Department at 330-332-7267 or Pastoral Care at 330-332-7574.

Reporting a 病人 Safety Concern: Concerns related to patient safety must be submitted in writing to 联合委员会(TJC) using one of the following methods:

  • At www.jointcommission.org, using the “Report a 病人 Safety Event” link in the “Action Center” on the home page of the website.

Reports of patient safety events to TJC must include the health care organization’s name, 街道地址, 城市和州. In the course of evaluating a report, TJC may share the information with the organization that is the subject of the report.
